Overview In this role you will deliver evidence-based community mental health treatment within the Nathaniel ACT Team, supporting recovery and community integration for adults with serious mental illness and criminal legal involvement. You will engage clients in outreach, provide psychoeducation, and advocate within the criminal justice system. Your work spans direct clinical services, coordination with support networks, and collaboration with a multidisciplinary team to reduce reliance on crisis services. Responsibilities- Conduct predominantly community-based treatment contacts ( 80%) and lead groups when appropriate
- Participate in daily team meetings addressing clinical and case management needs
- Complete documentation per OMH, DOHMH licensing guidelines, and Medicaid billing rules, plus CASES policies
- Collaborate on treatment planning, service coordination, and case management to promote recovery and stability
- Provide anti-racist, gender-responsive, trauma-informed, recovery-oriented, client-centered, evidence-based clinical services
- Administer required documentation and toxicology tests for clients with active AOT orders
- Provide 24-hour crisis intervention on-call coverage on a rotating basis
- Deliver culturally competent services per CASES policies
- Engage with clients' support networks and other providers to coordinate care
Key requirements- New York State license: LMHC, MHC-LP, LMSW, or LCSW
- Experience with individuals with serious mental illness and/or co-occurring substance use disorders
- Experience with evidence-based treatment models for substance use and mental illness
- Strong organizational skills, team orientation, and self-motivation
- Candidates with lived experience encouraged to apply
- Preferred: experience with individuals involved in the criminal legal system; Spanish fluency
