WHAT YOU'LL DO- Provide comprehensive adult primary care, in clinic and via telehealth
- Manage complex chronic conditions in older adults
- Lead daily team huddles on care plans and priorities
- Partner with care team members on integrated patient plans
- Manage patient communication and follow-ups
- Coordinate with emergency departments on continuity of care
- Document in Athena EHR
REQUIREMENTS- MD or DO, BC/BE in Internal Medicine
- Active Massachusetts license, or ability to obtain before start
- Strong evidence-based primary care skills
- Comfort with EHR and digital health tools
- Ability to connect with patients from diverse backgrounds
- Strong communication and documentation skills
PREFERRED- Experience with older adults in complex social settings
- Spanish proficiency
- Telehealth or remote patient monitoring experience
