Compensation: Base salary ranges from $225,000 to $265,000, depending on experience. Sign-on + relocation assistance are negotiable, plus a health center performance bonus.

Additional Info: Join a collaborative outpatient-only FM team with low call burden, protected admin time, and a schedule designed to support long-term physician sustainability. This position reports directly to the Chief Medical Officer and plays a vital role in delivering high-quality, comprehensive care within our FQHC.The PCP will serve as both an independent and collaborative member of the healthcare team, providing full-scope primary care services while supporting our mission to improve access to care for the communities we serve. Qualifications- Eligible for and able to obtain medical licensure in the state of Indiana- BE/BC in Internal Medicine- Current CPR certification- Ability to meet credentialing requirements of affiliated hospitals
