Position Overview
Country Inn Enhanced Living Center is seeking a compassionate, dependable, and resident-focused Licensed Practical Nurse (LPN) to join our team in providing quality care to residents in our assisted living community in rural Paulding County.
The LPN plays an important role in supporting residents' health, safety, comfort, and overall well-being. This position is responsible for providing nursing care, administering medications as authorized, monitoring resident conditions, and communicating with residents, families, healthcare providers, and facility staff.
Working collaboratively with the facility administrator and care team, the ideal candidate is attentive to residents' needs, demonstrates sound clinical judgment, and takes pride in delivering respectful, personalized care.
Responsibilities
- Provide nursing care and assistance to residents in accordance with established care plans, facility policies, and applicable regulations.
- Administer medications and treatments as authorized and in accordance with physician orders and applicable nursing regulations.
- Monitor residents' health status, recognize changes in condition, and report concerns promptly to the appropriate healthcare provider or supervisor.
- Document nursing assessments, medication administration, treatments, and resident care accurately and in a timely manner.
- Assist with developing, implementing, and updating resident care plans as appropriate to the role.
- Communicate effectively with residents, families, physicians, pharmacies, and other members of the healthcare team.
- Coordinate appointments, follow-up care, and other healthcare needs as required.
- Support residents with activities of daily living and personal care needs as appropriate.
- Follow infection prevention, medication safety, emergency response, and resident safety procedures.
- Maintain resident confidentiality and protect personal health information.
- Respond appropriately to resident concerns, changes in condition, and emergencies.
- Promote a respectful, supportive environment that preserves resident dignity, independence, and quality of life.
Required Qualifications
- Current, active Licensed Practical Nurse (LPN) license in the state of Ohio, in good standing.
- Knowledge of basic nursing practices, medication administration, and resident care procedures.
- Strong observation, communication, and documentation skills.
- Ability to recognize and report changes in resident health and condition.
- Compassionate, patient-centered approach to care.
- Ability to work collaboratively with residents, families, and facility staff.
Preferred Qualifications
- Previous nursing experience in assisted living, long-term care, skilled nursing, or another healthcare setting.
- Experience with medication administration and medication documentation.
- Familiarity with care plans, infection control, and resident safety protocols.
- Strong organizational skills and the ability to prioritize multiple resident care needs.
- Experience communicating with physicians, pharmacies, families, and interdisciplinary care teams.
