Home Health Nurse Practitioner

We are seeking experienced Nurse Practitioners (NPs) and Physician Assistants (PAs) for flexible Home Health Assessment opportunities.

Home Health Setting

Flexible Scheduling Full-Time or Part-Time Available

Daytime Hours Available 7 Days a Week

Competitive Compensation

Conduct pre-scheduled in-home health evaluations for Medicare Advantage, Medicaid, and select commercial health plan members.

Visits include:

• Comprehensive medical and medication history review
• Physical health assessment
• Completion of patient-specific documentation provided prior to each visit

This is a non-treatment role:

• No prescribing medications
• No ordering labs or diagnostic testing
• No modifying existing treatment plans

• Flexible scheduling options
• Average of 6 9 patient visits per day

Active NP or PA license in the state of practice

Board Certification

Medicare/Medicaid eligible

• Approximately 30-day credentialing process
