Position Summary
We are seeking a dedicated Licensed Practical Nurse (LPN) to join our multidisciplinary care team. The LPN will provide direct nursing care and ensure compliance with medical and behavioral health standards, while working under the direct supervision of a Registered Nurse (RN). This position plays a critical role in patient safety, comfort and progress throughout recovery. You'll represent the culture we're known for - calm, supportive, accountable and recovery-focused.
Key Responsibilities
- Provide direct nursing care to patients in medical detox and residential treatment under the supervision of the RN on shift and/or the Medical Director
- Monitor and document vital signs, withdrawal symptoms, and patient progress using standardized tools (COWS and CIWA); report observations for RN interpretation and review
- Administer prescribed medications and perform counts per company policy
- Review client charts for new doctor orders, medications, and transcription accuracy.
- Manage medication inventory, coordinate with pharmacy, and ensure timely delivery.
- Collect urinalysis samples and conduct breathalyzer tests per facility policy.
- Collaborate with multidisciplinary team members to deliver safe, compassionate, individualized care plans.
- Maintain accurate documentation in compliance with facility policies and state regulations.
- Monitor patients for changes in condition and promptly alert RN or provider.
- Recognize and report medical or behavioral emergencies, withdrawal complications, or medication reactions.
- Participate in staff meetings, shift change reports, and mandatory licensing/Joint Commission trainings.
- Be punctual and reliable for scheduled shifts
Qualifications
- Valid LPN license in Arizona with at least 1 year of experience
- Current CPR/First Aid (BCLS) certification (Health Care Provider preferred)
- Strong knowledge of medications and side effects
- Experience with order transcription, charting, medical assessments, admissions, and discharges.
- Excellent communication and de-escalation skills
- Knowledge of substance use and mental health disorder treatment
- Ability to pass drug screening and background check.
- Basic knowledge of 12-step programs; 2 years sobriety required if in recovery.
