Overview In this role you ensure compliant patient information review and training across branches to support high-quality home health care. You partner with leadership to educate Licensed Professionals on documentation requirements and regulatory standards, providing one-on-one coaching when needed. You help maintain strong relationships with referral sources, patients, and care teams while promoting consistent, quality patient care. This position emphasizes hands-on training, regulatory adherence, and cross-functional collaboration to enable safe, effective care delivery.

Responsibilities- Review and approve patient information submitted by Licensed Professionals (LPs) during start of care, recertification, resumption of care, or evaluation visits
- Educate LPs on documentation requirements to meet regulatory standards and keep staff updated on changes
- Provide one-on-one training when necessary to deepen LPs' understanding
- Promote quality patient care and maintain stable relationships with referral sources, patients, caregivers, and employees
Key requirements- Graduate of an approved nursing or physical therapy program
- Valid clinical license per state requirements
- Minimum of one year of experience
- Previous Medicare home health or hospice experience
- CPR certification
- Valid state driver's license and automobile liability insurance
- Basic technology skills; comfortable with mobile devices
- Preferred: experience with packet review/quality improvement; experience with coding; familiarity with Homecare Homebase (HCHB)
