Overview Inthis role you ensure accurate patient information and regulatory alignment across start of care, re-certification, and care evaluations. You partner with branch leadership to educate LPs on documentation requirements, driving consistent quality across the network. You promote strong relationships with referral sources, patients, and caregivers while supporting staff development. Responsibilities- Review and approve patient information submitted by Licensed Professionals during start of care, recertification, resumption of care, or evaluation visits
- Partner with branch leadership to educate and train LPs on documentation to meet regulatory standards and reflect changes
- Provide one on one training when needed
- Promote quality patient care and maintain stable relationships with referral sources, patients, caregivers, and employees
Key requirements- Graduate of an approved nursing or physical therapy program
- Valid clinical license per state requirements
- Minimum of 1 year of experience
- Experience in Medicare home health or hospice setting
- CPR certification
- Valid state driver license and automobile liability insurance
- Preferred: packet review/quality improvement experience
- Preferred: coding experience
- Basic technology skills including mobile devices
- Preferred: Homecare Homebase (HCHB) experience
- Demonstrated English communication skills (oral/written) and active listening
- Intermediate technology skills, including Microsoft Word and Excel
