Overview In this role you ensure accurate patient information and compliant documentation for start of care, re-certifications, and evaluations. You partner with branch leadership to educate LPs, provide focused training, and uphold regulatory standards. You'll support quality patient care and strengthen relationships with referral sources and care teams. Responsibilities- Review and approve patient information submitted by Licensed Professionals at key care moments
- Partner with branch leadership to educate LPs on documentation requirements to meet regulatory standards
- Provide one-on-one training to LPs when needed
- Promote quality patient care and maintain relationships with referral sources, patients, caregivers, and employees
Key requirements- Graduate of an approved school of professional nursing or physical therapy
- Valid clinical license per state requirements
- Minimum 1 year of experience
- Experience in Medicare home health or hospice setting
- CPR certification
- Valid state driver license
- Automobile liability insurance
- Basic demonstrated technology skills; Microsoft Office proficiency
- Ability to operate a mobile device
- In-office attendance essential (for non-remote roles)
- Preferred: packet review/quality improvement
- Preferred: coding
- Preferred: Homecare Homebase (HCHB) experience
- Fluency in English; strong written and oral communication
