Overview In this role you empower and assist vulnerable community members in navigating health care and social services, helping them maximize benefits and access needed resources. You'll work with the health services team in the field, including home and community settings, to support quality, cost-effective member care. You will engage in outreach to reconnect disconnected members and guide them through Medicaid eligibility and financial resources. The position involves local travel and cross-functional collaboration to address members' holistic needs.

Responsibilities- Advocate for members and support health care needs management
- Provide non-clinical paraprofessional support in the field (homes, shelters, provider offices)
- Help members schedule appointments, arrange transportation, manage prescriptions, and follow up on missed visits
- Assist members in accessing social services and community resources
- Reach out to locate and support disconnected members with special needs
- Conduct research using data and contacts to locate hard-to-reach members
- Participate in outreach activities (telephonic or face-to-face)
- Guide members to maintain Medicaid eligibility and access financial resources
Key requirements- At least 1 year of health care experience with underserved or special needs populations or equivalent education/experience
- Valid driver's license, reliable transportation, and adequate auto insurance for travel
- Ability to multi-task while communicating with members
- Excellent customer service, organizational and time-management skills
- Willingness to learn other programs and software systems
- Excellent verbal and written communication skills
- Proficiency in Microsoft Office and applicable software programs
