Growing together. -As a Per diem Hospice Registered Nurse, you will provide compassionate, patient-centered care by assessing needs, coordinating individualized care plans, managing symptoms, educating families, and collaborating with the clinical team. -Primary Responsibilities:Performs initial nursing assessment on new admissionsCompletes, coordinates, and revises the total plan of care and maintains continuity of patient care by collaborating with appropriate staffRegularly communicates patient progress to the clinical manager, physician, and care teamDevelops, prepares, and maintains individualized patient care progress records with accuracy, timeliness and according to care center policiesRecords pain/symptom management changes/outcomes as appropriateEducates families on patient condition, needs and careMakes referrals to other hospice care disciplines as needed, or documents rationale for not doing soPerforms on-call responsibilities and on-call services to patients/families as assignedPerforms other duties as assigned -You'll be rewarded and recognized for your performance in an environment that will challenge you and give you clear direction on what it takes to succeed in your role as well as provide development for other roles you may be interested in.Required Qualifications:Current RN license, specific to the state(s) you are assigned to workValid driver's license, reliable transportation, and liability insuranceCurrent CPR certification -Preferred Qualification:1+ years of nursing experience -Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements).
