Concierge Primary Care Physician (MD/DO, IM or FM) Appleton, WI $300K Base + Partner-Level Profit Sharing
A multidisciplinary medical center in Northeast Wisconsin is launching a membership-based concierge primary care program and is seeking an Internal Medicine or Family Medicine physician to care for a panel of roughly 300 to 600 patients. Visits run 30 to 60 minutes, with approximately 6 to 10 patients seen per day. You will also join collaborative-care meetings with the center s Functional Medicine team, helping guide care and develop treatment processes. If you are trained in endoscopy or colonoscopy, there is an opportunity to perform these procedures at the facility s ASC
Compensation and Schedule
- Base salary: $300,000
- Partner-level profit sharing starting in year one
- Up to $10,000 in relocation assistance
- 4 weeks PTO and a $2,500 CME allowance
- Group malpractice coverage ( 1M/ 3M) and a full benefits package
- Monday to Friday, 8 AM to 5 PM (negotiable); after-hours availability suited to a concierge practice
- Start: January to March 2027, depending on licensure and relocation; the employer helps with licensing and credentialing
What You'll Do
- Design and grow the concierge program: clinical standards, workflows, membership expectations and the patient experience
- Provide comprehensive, relationship-based primary care to a deliberately small panel
- Build individualized prevention, wellness, longevity and chronic-care plans for each patient
- Coordinate care with specialists and advanced practice providers in the same building
- Bring the primary care view to multidisciplinary Functional Medicine case discussions
- With the right training and privileges, perform endoscopy and colonoscopy and help build that service
Who Fits
- MD or DO, board certified or board eligible in Internal Medicine or Family Medicine (other primary care backgrounds considered)
- Eligible for a Wisconsin medical license
- Strong primary care skills and a real preference for longer visits and long-term patient relationships
- Concierge, direct primary care, executive health or preventive medicine experience is a plus, not a requirement
- Entrepreneurial and comfortable building a program rather than stepping into a finished one
- Endoscopy/colonoscopy training or experience is highly desirable (documentation of training and current competency needed for privileges)
What makes this opportunity unique
This is not another employed IM/FM slot measured in RVUs. Membership revenue replaces volume pressure, the smaller panel cuts the charting and inbox load, and you get to shape the program from day one with a profit share to match.
