Family Medicine Physician / Primary Care Provider

Position Highlights

Responsibilities
- Conduct comprehensive patient evaluations, histories, and physical examinations
- Diagnose and treat acute and chronic medical conditions
- Develop individualized treatment plans and provide patient education
- Order, interpret, and follow up on laboratory and diagnostic testing
- Prescribe medications and manage ongoing treatment plans
- Coordinate specialty referrals and continuity of care
- Complete accurate and timely medical documentation within the EHR
- Participate in quality improvement initiatives, peer review activities, and provider meetings
- Provide clinical guidance and supervision as required
- Maintain all professional licenses, certifications, and credentialing requirements
Qualifications
- MD or DO from an accredited medical school
- Board Certified or Board Eligible in Family Medicine preferred
- Active and unrestricted medical license in any U.S. state
- Active DEA registration
- Experience with electronic health records (EHR)
- Experience working with rural, underserved, or Native American populations preferred
- Knowledge of community-based healthcare and care coordination
- Strong communication and patient-centered care skills
Certifications Required
Preferred Qualifications
- Experience in outpatient primary care
- MAT/X-Waiver experience or willingness to obtain
- Experience with communicable disease management
- Familiarity with RPMS or similar EHR systems
