Requirements:
  • ABOS Board Certified or TRULY Board Eligible in Total Joint / Adult Reconstruction Orthopedic Surgeon (must become certified within 5 years) - REQUIRED no exceptions
  • Fellowship trained in Adult Reconstruction / Total Joint Arthroplasty PREFERRED
  • Valid IL & IA license or Active IMLC Willing to Obtain License - REQUIRED
  • Strong clinical and surgical skill set with a passion for total joints and revision work - REQUIRED
  • Hold current and unrestricted state and federal authority to prescribe and administer medications as necessary or appropriate to the field of practice - REQUIRED
  • DEA - REQUIRED
  • BLS Certification and maintain recertification - REQUIRED
  • Clean malpractice and license history - HIGHLY PREFERRED / PREFERRED
  • Does the provider require visa support - No J-1 or H-1B (Note in Additional Submission Details) - REQUIRED
  • Submit Self-Query NPDB pulled within 30 days from submission at time of presentation - REQUIRED
  • BLS
  • Board Certified
  • Board Eligible
