Join our dynamic healthcare team as a mobile Family Nurse Practitioner (FNP) for Maricopa County, where you will play a vital role in delivering comprehensive, patient-centered care across diverse settings.
As an energetic and motivated provider, you will utilize your advanced clinical skills to assess, diagnose, and manage a wide range of health conditions. This position offers the opportunity to work with the patient in their homes or large communities. Your expertise will contribute to improving health outcomes through compassionate care, innovative treatment approaches, and collaboration with multidisciplinary teams.
Responsibilities
Conduct thorough physical examinations and patient assessments to determine health status and develop individualized care plans.
Manage acute and chronic conditions utilizing evidence-based practices, including pain management, infectious disease care, and geriatrics.
Document clinical encounters accurately using EMR (Electronic Medical Records) systems like Epic, Cerner, or eClinicalWorks while ensuring compliance with HIPAA regulations.
Provide patient education on health coaching topics including nutrition, family planning, memory care, and post-acute discharge planning.
Collaborate with specialists in areas such as gastroenterology, dermatology (esthetic laser treatments), occupational health, hospice & palliative medicine, and trauma centers to coordinate comprehensive care.
Support patients across various environments
Experience
Valid Family Nurse Practitioner license with current certification from a recognized certifying body such as ANCC or AANP.
Demonstrated experience working in diverse healthcare settings including hospital medicine, emergency medicine, internal medicine, or behavioral health.
Proficiency with EMR/EHR systems for accurate medical documentation and coding (CPT/ICD).
Prior experience managing complex cases
Strong knowledge of physiology and anatomy to perform comprehensive physical exams and diagnostic evaluations effectively.
Familiarity with telehealth platforms to provide remote patient consultations when needed.
Ability to work collaboratively within multidisciplinary teams while demonstrating excellent communication skills.
