A skilled nursing facility serving Worcester and Milford, Massachusetts, is seeking an experienced MDS Coordinator to join its team. Key Responsibilities
- Complete and submit accurate, timely MDS assessments in compliance with CMS/RAI guidelines
- Coordinate the interdisciplinary team to ensure assessments, care plans, and documentation are completed on schedule
- Develop, review, and update resident care plans based on assessment findings
- Monitor documentation to support accurate PDPM classification and reimbursement
- Track assessment schedules and submission deadlines
- Stay current on federal and state regulatory requirements related to MDS
Qualifications
- Minimum of 1 year of MDS experience required
- Strong knowledge of the RAI process, PDPM, and CMS regulations
- Excellent organizational skills and attention to detail
- Ability to collaborate effectively with nursing, therapy, and administrative staff
