In this role, you'll provide skilled, compassionate nursing care to patients in their homes and hospice settings, supporting patients and families during one of life's most meaningful times. Position Offers
- Competitive pay: $70,000 $80,000 per year, depending on experience

Key Responsibilities
- Provide skilled nursing care to patients in their homes and hospice settings
- Perform comprehensive patient assessments and monitor changes in condition
- Develop and implement individualized plans of care
- Administer medications and treatments as ordered
- Educate and support patients and families
- Coordinate care with physicians, NPs, social workers, chaplains, aides, and other members of the interdisciplinary team
- Complete accurate clinical documentation
Qualifications
- Active Illinois RN license
