  • Reason for Coverage: The physician left the practice, relocating
  • Patient Population for Required Coverage: Adult
  • Number of Physicians in Practice: 2
  • Average Number of Patients Per Day - HOSPITAL: 12-15
  • Clinical Area: Oncology - Outpatient Setting, Oncology - Inpatient Setting, Hematology
  • Documentation System/EMR: EPIC
