  • Provider Type Needed: Pediatric Dentist or General Dentist with pediatric experience
  • Schedule:
    • October 12-16
    • 6:45am - 4pm
  • Provider must have a sedation permit

  Requirements

  • Active Arizona State License
  • DDS/DMD from an accredited dental school
  • Medicaid credentialed
  • Sedation permit
