In this role, you will: - Provide comprehensive primary care for patients across the lifespan.
- Diagnose and manage acute and chronic medical conditions.
- Perform wellness visits, preventive care, and chronic disease management.
- Educate patients on lifestyle modifications, nutrition, and preventive health.
- Order and interpret diagnostic tests and develop individualized treatment plans.
- Collaborate with physicians and the interdisciplinary team to ensure high-quality, coordinated care.
- Maintain accurate clinical documentation and support continuity of care.
We're looking for: - A Texas-licensed Family Nurse Practitioner (or eligible for Texas licensure).
- Experience or interest in family practice, primary care, chronic disease management, or integrative medicine.
- A compassionate, collaborative provider who values patient education and long-term relationships.
