Registered Nurse (RN) Case Manager

We re hiring a Registered Nurse (RN) Case Manager who wants a predictable weekday schedule, meaningful work with older adults, and the flexibility of a hybrid position.
You ll step into a community-based care management role focused on developing individualized care plans, coordinating services, and helping older adults remain safely in their homes. This is a care management position rather than a bedside nursing role, giving you meaningful autonomy with interdisciplinary support.

• You hold a clear and active Massachusetts RN license
• You have at least 2 years of RN experience
• Home care experience is preferred but not required
• You maintain current CPR certification

Compensation & Benefits
• Salary: $84,000 $108,000
