We are seeking a Hospice & Palliative Medicine Physician to join our team in Louisville and provide thoughtful, patient-centered care across inpatient and community settings.
The Opportunity
Provide attending and consulting physician services for hospice and palliative care patients.
Care for patients at the Hosparus Inpatient Care Center and designated community locations.
Collaborate with interdisciplinary teams, community physicians, nurse practitioners, and families to coordinate individualized care.
Conduct medically necessary patient visits, consultations, and hospice certifications and recertifications.
Participate in rotating inpatient and organization-wide night and weekend call coverage.
Contribute to physician education, community outreach, clinical quality initiatives, and the development of strong relationships with healthcare partners.
Qualifications
MD or DO required.
Kentucky and Indiana medical licensure, or eligibility to obtain both, including applicable DEA registrations and controlled substance permits.
Fellowship training in Hospice and Palliative Medicine or at least two years of post-residency medical experience .
Board certification or board eligibility, with certification expected within five years of employment.
Experience in hospice, palliative care, primary care, hospital medicine, or geriatrics preferred.
Hospice and Palliative Medicine certification preferred.
