  • ASAP Ongoing
Shifts:
  • Monday Friday 8a-5p
Setting:
  • Outpatient facility
  • Value Based Care model
  • Seeing ages 65+
  • Average patients per day: 10-13
  • Patient panel max of 400
Support staff:
  • Each provider has their own care team/scheduler, MA, referral specialist, Nurse, Case Manager, Social Worker and Outreach Specialist
  • EKGs and labs are performed in center
EMR:
  • Epic
Travel accommodations
  • Covered by facility
Credentialing timeline:
  • 2-3 weeks
Requirements:
  • Active ALlicense
  • Active ALDEA
  • Active BC
