APP Post Acute Care opening in Chattanooga, TNKnoxville 112m, Nashville 134mSeeking a quality-driven NP or PA to join our Post-acute Care Team in the Chattanooga, Tennessee, area. This is a full-time position (4 days/week) rounding Friday - Monday in skilled nursing facilities, with excellent support and training, autonomy, and earning potential.

Key Responsibilities- Perform comprehensive patient assessments, including medical histories and physical exams
- Develop, implement, and adjust individualized care plans based on patient needs
- Monitor progress and coordinate care to optimize outcomes
- Collaborate closely with physicians, nursing staff, and other healthcare professionals in a team approach
- Educate patients and families on health management and preventive strategies
- Maintain accurate, timely medical records in compliance with regulations
Compensation and Benefits- Compensation is fee for service with an estimated compensation range of $93,850 to $111,021 annually and no cap on productivity income potential.

Qualifications- Ability to commute/relocate: Ooltewah, Cleveland, East Ridge, Hixson, TN
- Work location: in-person
- Tennessee NP or PA license and DEA
- Experience in post-acute, acute, ED, or clinic settings preferred, but not required; new grads welcome to apply
- Strong clinical foundation and knowledge of healthcare regulatory standards
