Opportunity Highlights

- Pay: $140,000 $160,000/year
- $10K+ Sign-On Bonus

- Full-Time Monday Friday 8 AM 5 PM
- ️ 8 10 clinic patients/day 5 6 home visits/day

Daily Responsibilities
As an FNP, you ll evaluate and manage senior and high-risk patients, conduct wellness exams, review medications, perform screenings, develop individualized care plans, and coordinate follow-up care. You ll also provide patient education, support transitions after hospital discharge, collaborate with an interdisciplinary care team, and provide care in both clinic and home settings.
Requirements
- Current Oregon NP License & Certification
- 2+ years NP experience in Primary Care, Family Medicine, Internal Medicine, Gerontology or Palliative Care
- DEA/Prescriptive Authority
- CPR/BLS certification
- Oregon Driver s License
