Required Procedures: No procedures outside of radiation oncologist-consults, simulations, follow-ups in clinic operations. No surgery etc.
Required clinical activity in the last 24 months (case logs): Applicant must provide documentation (case list) of provision of clinical services (50 patients) representative of the scope and complexity of the privileges requested during the previous 24 months. Case list must include diagnosis or procedure and date, and facility where performed.
Will provider work solo? Yes
Licensed Candidate: Yes, Washington license or IMLC with LOQ required in order to meet our desired start date. Applicant will need to have at least applied for the Washington license via IMLC at time of pre-app
Required certifications (Must be from American Heart Association or American Red Cross as applicable): BLS

Additional dept. details:
The department is staffed with 4 Radiation Therapists, 1 dosimetrist, 1 physicist, 2 support office staff. No MA or nurses on staff but directly attached to Medical Oncology. There is a CT simulator on site respiratory gating. The planning equipment is primarily Eclipse, and we use MIM for all 4D.
