Since 2008, Lifespan Therapy has empowered qualified clinicians to connect with homebound geriatric patients, having served over 100,000 individuals through our innovative app-based network. And now, we are officially expanding outside of California and bringing our clinician-first platform to Arizona!

Are you an experienced home health clinician, or are you looking to transition or supplement your income from outpatient, SNF, or hospice settings? We are looking for licensed Physical Therapists (PT) to join our growing network in Maricopa County. We're here to support you in delivering patient-centered care with unparalleled flexibility and independence.

Why Choose Lifespan Therapy?
  • Ultimate Flexibility: Our app gives you total control over your schedule and preferred work locations. Work when and where you want, supported by a team that's always there.
  • Workflow Made Simple: Our app handles credentials, scheduling, communication, and invoicing-giving you the tools you need to manage your day efficiently.
  • Focus on Patient Progress: With us managing referrals, authorizations, and billing, you can devote your full attention to implementing top-tier plans of care.
  • Compliant Charting: Our integrated EMR, TherapySync, makes documenting patient progress simple and compliant, allowing for seamless charting.
  • Expert Guidance: Receive consultative services to ensure your documentation is compliant and meets the highest professional standards.
  • Hassle-Free Payments: Earn reliable bi-monthly direct deposits.
What You'll Do
  • Provide Skilled Home Care: Deliver individualized physical or occupational therapy services (evaluations and treatments) in the patient's home environment.
  • Perform Therapeutic Interventions: Focus on functional strengthening, gait training, ADL retraining, balance, and fall risk reduction to help patients reach their independence goals.
  • Report & Collaborate on Patient Progress: Document patient responses to treatment and maintain accurate records to ensure a coordinated approach.
  • Manage Your Caseload: Set your own schedule and visit cadence based on your personal availability and the patient's plan of care.
  • Document Efficiently: Use one simple EMR system for all notes (with tools available to minimize documentation time!).
What You'll Need
  • Valid Arizona Physical Therapy professional license
  • CPR or BLS certification
  • Valid driver's license and auto insurance
  • Physical + TB test (within the last year)
  • Professional liability insurance
  • Business license (if applicable)
Where You'll Be Working

By joining our inaugural Arizona team, you have the unique opportunity to help establish our footprint across the booming, fast-paced Phoenix metropolitan area. This role offers ultimate geographic flexibility. You can choose your routes based on where you decide, where you like to drive, and what you enjoy-whether you prefer The East Valley, West Valley, or are North of the area-ensuring your coverage is as adaptable and flexible as you need it to be. The Vibe: Navigate the energy of The Valley. Whether you prefer the East Valley, West Valley, or are North of the area, your routes can span from the scenic mountain backdrops of Scottsdale and Phoenix to the rapidly growing master-planned communities, keeping your work as adaptable and flexible as you need it to be. Enjoy great local coffee culture and incredible winter weather while keeping your routes tight and efficient. The Community: You will be part of a dynamic, diverse community. Because of the region's demographics, Spanish proficiency is a major asset to connect with our patient population, though it is not required.
