You ll step into a nursing role providing care to seniors in outpatient and community-based settings. You ll conduct assessments, coordinate individualized care plans, manage medications, and help older adults remain safely and independently in their homes. You ll enjoy clinical autonomy while collaborating with a supportive interdisciplinary healthcare team.

• You hold an active Massachusetts Registered Nurse (RN) license
• You have at least 2 years of experience caring for geriatric patients
• You re comfortable conducting nursing assessments and developing individualized care plans
• You have experience with medication management and care coordination
• You re willing to travel locally using your personal vehicle
• You have strong clinical judgment, communication, and teamwork skills
• You value compassionate, patient-centered care and helping seniors maintain their independence
Compensation & Benefits
• Annual salary: $80,000 $104,000
