Nurse - Case Manager

Job Title: RN Case Manager/Case Management Coordinator Functional Group: Case Management Duration: 3 months (Extendable or FT hire based on performance) Schedule: M-F 8am-4:30pm & 8 hours per day, 40 hours in a week (OT is also available time and a half) Requirements: At least five (5) years experience Knowledge of managed care admission process (i.e. verification of benefits, admissions notification) Experience managing patient caseloads of approximately 18-25 patients daily Computer skills and a working knowledge of Word, Excel and Access Previous experience with discharge planning or home care experience Prior experience as a Case Manager or Utilization Reviewer
