An established outpatient primary care practice is seeking a Family Nurse Practitioner (FNP) or Physician Assistant (PA) to join a collaborative healthcare team serving a diverse patient population in Central California.

Key Responsibilities
- Provide comprehensive primary and preventive care to patients of all ages.
- Evaluate, diagnose, and manage acute and chronic medical conditions.
- Conduct physical examinations and wellness visits.
- Order and interpret diagnostic tests and laboratory results.
- Develop evidence-based treatment plans.
- Prescribe medications and therapies within scope of practice.
- Coordinate referrals and follow-up care with specialists.
- Educate patients on disease prevention and healthy lifestyle choices.
- Maintain accurate and timely electronic medical records.
- Collaborate with physicians, nurses, and interdisciplinary healthcare teams.
Qualifications
Required
- Graduate of an accredited Nurse Practitioner or Physician Assistant program.
- Active California NP or PA license, or ability to obtain licensure.
- Strong clinical assessment and patient management skills.
- Excellent communication and interpersonal abilities.
Preferred
- 1+ year of experience in Family Practice, Primary Care, Internal Medicine, Community Health, Urgent Care, or Outpatient Medicine.
- Experience managing chronic diseases and preventive care services.
- Bilingual skills are a plus but not required.
