Weekend Baylor LPN - The Madison Health and Rehab - 12 hour shifts

The Weekend Baylor LPN provides nursing care for a mixed rehabilitation and long-term care resident population at The Madison Health and Rehab during scheduled 12-hour weekend shifts. Reporting to the Director of Nursing, this role focuses on medication passes, treatments, resident monitoring, and accurate documentation using PointClickCare.

$25.50 per hour

$ 2.00 shift differential from 3p - 7a

Work a 12 hour shift Saturday, 12 hour shift Sunday, receive 6 additional hours

Insurance and benefits provided after 60 days

Responsibilities

• Conduct resident assessments and monitor clinical status.
• Administer medications during scheduled medication passes.
• Deliver prescribed treatments during 12-hour weekend shifts.
• Measure and document vital signs.
• Complete accurate chart documentation in PointClickCare.
• Monitor residents and report clinical changes appropriately.
• Collaborate with the nursing and care team.

Required Qualifications

• Active Licensed Practical Nurse (LPN) license.
• Basic Life Support (BLS) certification.

Preferred Qualifications

• Medication administration experience.
• Clinical documentation experience.
• Vital signs monitoring experience.
• Wound care experience.
• Electronic health records experience, preferably PointClickCare.
• Knowledge of infection control practices.
• Patient assessment skills.
• Strong communication and time management skills.
