- Provide hospice nurse practitioner services to patients in the Austin area.
- Complete approximately four patient visits per day.
- Deliver care during a consistent Monday Friday, 8:00 AM 5:00 PM schedule.
- Evaluate patient needs and support hospice-focused care planning.
- Provide clinical care aligned with hospice services and patient goals.
- Communicate with the care team to support continuity of care.
- Document patient visits and clinical services as required.
