TRULY Board Eligible or Board Certified in Neonatal-Perinatal Medicine REQUIRED
Wisconsin Medical License (or willingness to obtain) REQUIRED
Clean Malpractice/License History (HIGHLY PREFERED)
Neonatology experience REQUIRED (experienced candidates only)/li>
Does the provider require visa support (J1/H1B)? REQUIRED to specify
Most recent NPDB Self Query Report (within 30 days active) - REQUIRED no exemption, required at presentation
DEA (must be obtained prior to start) - (note in "Additional Submission Details" field) REQUIRED no exemptions
