$34-$36/Hour 3-Day Workweek 4 Days Off
About the Position
Unify Home Care is a locally owned, growing home care company seeking compassionate, dependable Licensed Practical Nurses (LPNs) to join our team in Jackson, Wyoming.
We provide personalized, one-on-one nursing care in patients' homes, allowing our nurses to build meaningful relationships while making a difference in their patients' everyday lives.
We're looking for nurses who enjoy hands-on patient care, take pride in their work, and appreciate a supportive environment with direct communication from the owner.
Available Positions & Pay
We are currently hiring for two separate LPN positions, each offering a consistent three-day workweek.
Weekday LPN - $34/hour
- Monday, Tuesday, and Wednesday
- 8:30 AM - 9:00 PM
Weekend LPN - $36/hour
- Friday, Saturday, and Sunday
- 8:30 AM - 9:00 PM
- Includes a $2/hour weekend differential
Both positions offer:
- Three scheduled shifts per week (37.5 scheduled hours before any unpaid breaks)
- Four days off each week
- Consistent scheduling with established patients
- Hourly pay, not per patient visit
- No overnight shifts
- Holiday shifts are not required
Responsibilities
- Provide individualized nursing care in patients' private residences within the LPN scope of practice.
- Monitor vital signs, patient conditions, and changes in health status.
- Administer prescribed medications and treatments as permitted by applicable nursing regulations.
- Assist with activities of daily living, personal care, hygiene, and mobility.
- Support patients with daily routines, safety, comfort, and overall well-being.
- Recognize and promptly report changes in patient conditions.
- Communicate with patients, families, healthcare providers, and the nursing team.
- Complete accurate and timely nursing documentation.
- Follow individualized care plans, physician orders, agency policies, and applicable nursing supervision requirements.
Qualifications
- Active LPN license in Wyoming or a valid multistate nursing license authorizing practice in Wyoming.
- Current CPR certification.
- Valid driver's license and reliable transportation.
- Strong practical nursing skills and attention to detail.
- Ability to work independently within the LPN scope of practice while following appropriate clinical direction.
- Dependable, compassionate, and professional attitude.
- Excellent communication and organizational skills.
- Ability to successfully complete required background screening and drug testing.
Home care experience is preferred but not required! We welcome LPNs from a variety of nursing backgrounds who are passionate about providing quality patient care.
