Family Nurse Practitioner Onsite Employee Health
Windsor/DeForest, WI December 14, 2026-April 5, 2027 Day shift

Mint Workforce is hiring a Family Nurse Practitioner (FNP) for maternity leave coverage at an onsite employee health center serving a well-established convenience retail organization.
Care for employees and eligible dependents ages 2 and older with 30-minute office visits, 45-minute annual physicals, and dedicated daily administrative time.
You'll work alongside another onsite NP in a collaborative setting offering behavioral health, physical therapy, care management, and laboratory services.
Position Details
- Location: Windsor/DeForest area
- Contract: January 4th 2026-May 5th, 2027
- Clinic Hours:
- Monday-Tuesday: 6:30AM - 4:30PM
- Wednesday-Thursday: 11:00AM - 7:00PM
- Friday: 8:00AM-12:00PM
- Setting: Onsite employee health center
- EHR: Epic
- Lunch: 30 minutes
- Administrative time: 30 minutes at the end of each day
- Option to take the week of December 21 off
- Training planned before the current provider begins maternity leave
What You'll Do
- Provide family medicine, preventive, acute, and chronic disease care
- Address occupational health needs
- Assess, diagnose, and develop individualized treatment plans
- Provide patient education and coordinate referrals
- Collaborate with the onsite multidisciplinary team
- Maintain accurate documentation in Epic
- Deliver a welcoming, respectful, and positive patient experience
What We're Looking For
- Family Nurse Practitioner with active Wisconsin licensure and current FNP board certification
- Experience in primary care, acute care, and occupational health
- Comfortable treating adults and children ages 2+
- Strong clinical judgment, communication, and teamwork
- Commitment to patient-centered care and exceptional customer service
- Epic experience preferred
