Compensation
Local Candidates
- Pay Rate: $45/hour on W-2
Travel Candidates
- Gross Weekly Pay: $2,049/week
- Weekly Non-Taxable Stipend: $1,246/week
- Taxable W-2 Pay: $20/hour
Travel compensation is subject to applicable eligibility requirements.
Position Overview
Hartford HealthCare at Home is seeking an experienced Licensed Practical Nurse (LPN) - Home Health for a 13-week contract assignment based out of Bridgeport, Connecticut.
The LPN will provide skilled nursing services to patients in their homes throughout the assigned service territory. The ideal candidate must have previous home health experience and familiarity with Homecare Homebase (HCHB). OASIS experience is also requested as part of the candidate submission process.
This position offers a 40-hour workweek, mileage reimbursement, and a daytime schedule. Candidates should be comfortable traveling between patient homes and managing their assigned caseload while maintaining accurate clinical documentation.
Key Responsibilities
- Provide skilled nursing care to patients in their homes in accordance with established care plans and provider orders.
- Monitor patients' health status, assess symptoms, and report changes in condition to the supervising RN or appropriate healthcare provider.
- Administer medications and treatments within the LPN's scope of practice and applicable regulations.
- Assist with patient assessments, care coordination, and implementation of individualized nursing care plans.
- Educate patients and caregivers on medication management, treatment plans, disease management, and home safety.
- Document nursing visits, interventions, patient responses, and clinical findings accurately in the Homecare Homebase (HCHB) system.
- Collaborate with registered nurses, physicians, therapists, and other members of the home health care team.
- Support continuity of care by communicating patient progress and concerns promptly.
- Follow infection prevention, patient safety, and regulatory guidelines.
- Manage daily patient visits and meet the expected productivity requirements.
- Travel throughout the assigned service territory to provide in-home nursing care.
Required Qualifications
- Active and valid LPN license appropriate for practice in Connecticut.
- Previous home health nursing experience required.
- Experience with Homecare Homebase (HCHB) preferred and strongly desired.
- Familiarity with electronic medical records and clinical documentation.
- Strong patient assessment, communication, and organizational skills.
- Ability to work independently in a home health setting.
- Reliable transportation and ability to travel between patient homes.
- Ability to manage a patient caseload and meet productivity expectations.
Preferred Experience:
- Homecare Homebase (HCHB) EMR experience.
- OASIS documentation and assessment familiarity.
- Previous experience providing nursing care to patients in their homes.
