Position Overview
Provide in-home health assessments for adult/geriatric patients. Documentation is completed on a provided tablet-based platform (as referenced in the Order details).
Statewide Opportunities
Opportunities are available throughout most counties statewide. Assignments are scheduled based on provider availability and county demand.
Schedule
Monday - Sunday, 7:00 AM - 7:00 PM
Typical volume: 5-8 assessments per day
Providers must have at least one weekday available (weekend-only availability is not accepted)
Requirements
Active, unrestricted license appropriate to role in the state of practice
National certification required for NP/PA
NPI required
CAQH profile required (as referenced in the Order details)
Travel and Lodging
Travel and lodging may be available based on eligibility criteria referenced in the Order details.
Candidates must have at least two years of clinical experience within the past five years to qualify.
