Job Title: Nurse Practitioner Wound Care & Primary Care (Long-Term Care)
Location: Kettering, OH
Schedule: Monday Friday 8:00 AM 5:00 PM
Position Overview
We are seeking a Nurse Practitioner to provide wound care and primary medical services to residents in skilled nursing and long-term care facilities. This role focuses on delivering high-quality, patient-centered care while collaborating with interdisciplinary teams and facility staff to manage complex and chronic conditions.
Key Responsibilities
Deliver wound care and primary care services to long-term care residents
Assess, diagnose, and treat acute and chronic conditions
Prescribe medications, including controlled substances, in accordance with Ohio law
Order, review, and follow up on diagnostic tests and treatments
Stage pressure injuries according to NPIAP guidelines
Manage complex wounds (diabetic, arterial, venous, mixed etiology, Kennedy ulcers)
Utilize advanced wound care products (collagens, alginates, foams, silver dressings, etc.)
Use wound vacs and pressure-relief equipment as appropriate
Communicate regularly with nursing staff regarding patient status and care plans
Collaborate closely with primary care physicians and interdisciplinary teams
Document visits and care plans accurately within EMR systems
Required Qualifications
Active, unrestricted Nurse Practitioner license in Ohio
Board Certification
Clinical background in adult, family, or geriatric care
Experience treating chronic or complex wounds
Ability to manage patients with multiple comorbidities
Experience in long-term care settings
Proficiency with EMR systems
Strong collaboration and communication skills
