Schedule: Full-Time Clinic Hours, Including Every Other Weekend

Position Summary
The Licensed Practical Nurse (LPN) / Licensed Vocational Nurse (LVN) will be an integral member of the clinical team, delivering high-quality, evidence-based patient care in a fast-paced outpatient healthcare environment. Responsibilities include assessing patient needs, administering medications and treatments, performing clinical testing, documenting patient information, collaborating with healthcare professionals, and advocating for patients to support positive health outcomes.
Candidates should have experience in primary care, urgent care, or emergency room settings, along with strong clinical assessment, communication, and electronic medical record (EMR) documentation skills.

Key Responsibilities
- Administer oral, injectable, intravenous, and topical medications and treatments within the scope of licensure, training, and applicable protocols.
- Perform routine patient assessments and monitor overall health and well-being.
- Measure, evaluate, and document vital signs, including blood pressure, heart rate, respiratory rate, and temperature, and report abnormalities as appropriate.
- Assess patient conditions, monitor pain levels, and document responses to treatment.
- Perform phlebotomy, administer vaccines, and conduct point-of-care diagnostic testing.
- Review medications and identify potential side effects, interactions, and medication-related concerns for appropriate follow-up.
- Collect and accurately document patient medical histories, including illnesses, medications, allergies, surgical history, and family history.
- Assist with mental health screenings, including depression screening and assessments of mood, behavior, and cognitive function, as appropriate to the role and clinical protocols.
- Support patient access to healthcare services and provide education on medication administration, self-care, and disease management.
- Connect patients with appropriate community resources and healthcare services when needed.
- Communicate effectively with patients, families, and other healthcare professionals.
- Maintain a clean, organized, and adequately stocked clinic environment.
- Follow established clinical protocols, documentation standards, safety requirements, and applicable laws and regulations.
Required Qualifications
- Current, full, active, and unrestricted LPN/LVN license in a U.S. state, territory, commonwealth, or the District of Columbia.
- Minimum of one year of related work experience in a healthcare setting.
- Previous experience in primary care, urgent care, or emergency room settings.
- Experience with phlebotomy, vaccine administration, and point-of-care testing.
- Proficiency with electronic medical record (EMR) systems; Epic experience preferred.
- Current Basic Life Support (BLS) certification meeting the required validity period.
- Verifiable high school diploma or GED.
- Ability to complete the initial three-week training and orientation period.
- Availability to work the required full-time clinic schedule, including every other weekend and some major holidays.
