Beaverton, Oregon

Job description

Job Description Job Description

# Licensed Practical Nurse (LPN) Care Manager - Mobile Primary Care

Senior Care Northwest

Full-Time Monday-Friday Approximately 9:00 AM-5:00 PM

Pay: $32-$40 per hour, depending on experience

Location: Portland / Beaverton / Hillsboro and surrounding areas

Senior Care Northwest is an independent mobile primary care practice providing medical care to older adults where they live, including assisted living, memory care, skilled nursing, and independent living communities.

We are seeking an experienced Licensed Practical Nurse (LPN) to join our growing mobile clinical team.

This is a field-based nursing position working closely with a Nurse Practitioner to provide primary care throughout the Portland metro area, with a particular focus on Washington County.

The LPN will travel to senior living communities, participate in scheduled primary care visits, perform focused nursing assessments and phlebotomy, gather clinical information, communicate changes in condition, coordinate follow-up care, and help manage the provider's daily clinical workflow.

This is not a traditional clinic or bedside nursing position. The right candidate will be comfortable working independently in the community, managing multiple priorities, communicating closely with our provider team, and taking ownership of patient care tasks through completion.

Responsibilities

- Travel to patient homes and senior living communities throughout Portland, Beaverton, Hillsboro, and surrounding areas

- Work closely with a Nurse Practitioner in support of an established primary care panel

- Prepare patients for scheduled primary care visits

- Obtain and document vital signs, including blood pressure, pulse, respiratory rate, temperature, oxygen saturation, and weight

- Perform focused nursing assessments within LPN scope and communicate relevant findings to the Nurse Practitioner

- Recognize changes in condition and promptly escalate clinical concerns

- Perform venipuncture and collect laboratory specimens

- Label, process, and coordinate transport of laboratory specimens

- Obtain urine and other specimens as ordered

- Assist with examinations, minor procedures, wound evaluation, and other clinical care as appropriate

- Review medication lists and identify discrepancies between facility records, patient medications, pharmacy records, and the medical chart

- Gather clinical information from patients, caregivers, nurses, medication aides, facility staff, and family members

- Communicate provider orders and follow-up needs to facility staff

- Assist with implementation of provider treatment plans and medical orders

- Coordinate laboratory testing, imaging, referrals, DME, home health, pharmacy requests, and other follow-up care

- Monitor outstanding clinical tasks and ensure appropriate follow-through

- Review incoming laboratory results, messages, and clinical information and bring relevant concerns to the provider's attention

- Prepare charts and gather relevant records prior to upcoming visits

- Document clinical information accurately and promptly in the electronic medical record

- Maintain communication with the provider and clinical team throughout the workday

- Assist with occasional telehealth visits when appropriate

- Help maintain clinical supplies and equipment used in the field

- Participate in communication with senior living nursing staff regarding changes in patient condition and treatment plans

- Assist with other nursing and clinical tasks appropriate to LPN scope as patient and practice needs arise

What We're Looking For

We are looking for an LPN who is clinically capable, dependable, organized, and comfortable practicing with a significant degree of independence while maintaining close communication with the supervising provider.

The ideal candidate is someone who can enter a senior living community or patient's home, quickly understand the situation, gather useful clinical information, identify concerns, communicate effectively with the provider, and help move the patient's care forward.

Required Qualifications

- Current, unrestricted Oregon LPN license

- Valid driver's license and reliable transportation

- Ability and willingness to travel throughout the Portland metro area during the workday

- Strong nursing assessment and clinical observation skills

- Ability to recognize significant changes in patient condition and escalate concerns appropriately

- Strong written and verbal communication skills

- Comfort working independently in community-based settings

- Strong organizational and follow-through skills

- Comfort using electronic medical records, smartphones, tablets, and other healthcare technology

- Consistent punctuality and reliability

Strongly Preferred

- Proficiency and confidence performing venipuncture

- Experience working with older adults

- Experience in primary care, home health, assisted living, skilled nursing, geriatrics, or other community-based healthcare settings

- Experience caring for medically complex patients

- Experience communicating with providers regarding changes in condition

- Familiarity with medication reconciliation

- Experience coordinating laboratory testing, imaging, referrals, DME, home health, or other outpatient services

Who Thrives in This Role?

This position is particularly well suited for a nurse who:

- Enjoys developing a close working relationship with a provider

- Likes getting to know a consistent panel of patients

- Prefers community-based care over working in the same facility every day

- Enjoys older adults and medically complex patients

- Is comfortable making observations and communicating clinical concerns

- Takes ownership of tasks and follows them through to completion

- Notices what needs attention without waiting for every task to be individually assigned

- Uses unscheduled time productively by preparing upcoming visits, reviewing outstanding tasks, following up on orders, organizing supplies, or assisting other members of the team

- Is comfortable adapting when schedules or patient needs change

- Values autonomy while also communicating frequently with the clinical team

- Wants to play a meaningful role in a patient's ongoing primary care rather than providing episodic task-based care

Because we are a small practice, teamwork and initiative are important. We expect team members to remain engaged during the workday and to look for opportunities to move patient care and practice priorities forward.

Schedule & Compensation

- Full-time

- Monday-Friday

- Approximately 9:00 AM-5:00 PM

- $32-$40 per hour , depending on experience and clinical proficiency

- Paid travel time between patient locations during the workday

- Mileage reimbursement for business travel

- 75% employer-paid health insurance

- 401(k) with 4% employer match

- Primarily daytime weekday work

- No overnight shifts

About Senior Care Northwest

Senior Care Northwest is an independent mobile primary care practice focused on caring for older adults where they live.

Our providers care for patients in assisted living, memory care, skilled nursing, independent living, through telehealth, and in our clinic.

Our mobile model allows our nurses to become closely involved in the ongoing care of their patients. Rather than simply completing isolated nursing tasks, our clinical staff help providers identify changes in condition, coordinate care, solve problems, and ensure treatment plans are successfully carried out.

As a small and growing practice, we offer more independence and variety than many traditional medical settings. We value team members who are clinically capable, dependable, adaptable, organized, communicative, and willing to take ownership of their work.

As the practice continues to grow, we anticipate expanding our nursing and clinical support team and developing increasingly specialized roles.

If you are an experienced LPN who enjoys primary care, older adults, community-based nursing, phlebotomy, and working closely with a provider, we encourage you to apply!
