Conduct scheduled and clinically indicated follow-up visits to monitor progress, treatment response, medication effectiveness, adverse effects, and patient safety.

Assess and manage medication treatment, including dose recommendations or adjustments, orders, and prescriptions when authorized by the provider's scope and credentials and applicable OTP requirements.

Order, review, interpret, and follow up on relevant laboratory tests and toxicology results; document clinical decisions and communicate findings as appropriate.

Identify acute medical concerns, psychiatric symptoms, substance use risks, and other conditions requiring further evaluation, referral, escalation, or emergency intervention.

Provide patient education on treatment, medication safety, overdose prevention, and available community health resources.

Coordinate with nursing, counseling, case management, and administrative staff to address patient needs and continuity of care.

Consult with the medical director on complex cases and follow established escalation procedures.

Maintain accurate, complete, and timely records in the electronic medical record; aim to complete visit documentation by the end of the workday whenever practicable and comply with Thrive's documentation deadlines.

Participate in clinical meetings, quality improvement, policy implementation, and other reasonable duties related to patient care.

Excellent verbal and written communication and patient-centered interpersonal skills.

Sound clinical judgment, analytical and problem-solving skills, and attention to detail.

Ability to use electronic medical records and maintain patient confidentiality.

Education, Licensure, and Experience

Graduation from an accredited educational program qualifying the candidate as a Nurse Practitioner or Physician Assistant.

Current, unrestricted Colorado licensure or other applicable state authorization to practice as an NP or PA, together with all required professional credentials and certifications for the role.

Appropriate prescriptive authority and controlled-substance registration(s), when required for assigned duties.

Experience in addiction medicine, opioid use disorder treatment, behavioral health, or related outpatient care is preferred.

Ability to practice in accordance with applicable professional, state, federal, and opioid treatment program requirements.
