We offer a wide spectrum of services including Family Medicine, Primary Care, Adult Care (No pediatrics) In addition we offer Chiropractic Care, Neuromuscular Therapy, and Physical Therapy.
Provide comprehensive primary care to adult patients in a general family practice, with an emphasis on prevention, chronic disease management, acute same-day visits, and women's health. Collaborate with assigned physicians and a multidisciplinary care team under a collaborative practice agreement, consulting physicians on complex cases.
Responsibilities
- Assess adult patients and develop differential diagnoses.
- Diagnose conditions and create individualized treatment plans.
- Manage prescriptions in accordance with the collaborative practice agreement.
- Deliver preventive care and conduct health screenings.
- Manage chronic diseases and provide acute, same-day care.
- Provide women's health services.
- Educate patients on conditions, treatments, and preventive health.
- Coordinate care with assigned physicians and the multidisciplinary care team.
- Maintain accurate clinical documentation in electronic health records.
- Consult physicians regarding complex cases.
Required Qualifications
- Active Nurse Practitioner license.
- At least 2 years of experience in family practice nursing.
- Knowledge of family medicine, differential diagnosis, pharmacology, chronic disease management, and preventive care.
- Proficiency with electronic health records.
- Strong clinical judgment and patient communication skills.
