About the Role: AlwaysNear Home Care LLC is looking for a compassionate and dedicated PRN Licensed Practical Nurse (LPN) to join our growing team in Atlanta, GA. Responsibilities: - Provide direct patient care including wound care, medication administration, and vital sign monitoring
- Develop and follow individualized patient care plans in coordination with RNs and physicians
- Observe and document patient conditions, progress, and any changes in health status
- Assist patients with activities of daily living (ADLs) and personal hygiene as needed
- Communicate effectively with patients, families, and the broader care team regarding treatment plans
- Ensure compliance with all state, federal, and agency regulations and standards of care
- Provide patient and family education on medications, treatments, and home health management
Requirements: - Active Licensed Practical Nurse (LPN) license in the state of Georgia
- Current CPR/BLS certification required
- Prior home health, long-term care, or clinical nursing experience preferred
- Strong knowledge of medication administration, wound care, and patient assessment
- Excellent communication, organizational, and time-management skills
- Reliable transportation and valid driver's license for home visit travel throughout Atlanta, GA
- Compassionate, patient-focused demeanor with a commitment to quality care
- Pediatric Experience is a plus
