  • Clean malpractice and license history - HIGHLY PREFERRED
  • Most recent NPDB Self Query Report (within 30 days active) - REQUIRED no exemption, required at presentation
  • Does provider already live in the area or they willing to relocate? (note in "Additional Submission Details" field) - REQUIRED
  • Does the provider require visa support - J1 or H1B? (note in "Additional Submission Details" field) - REQUIRED
  • DEA (must be obtained prior to start) - (note in "Additional Submission Details" field) REQUIRED no exemptions
  • CSR/CDS (must be obtained prior to start) - (note in "Additional Submission Details" field) REQUIRED no exemptions
  • Board Certified (BC) or Board Eligible (BE) in Neonatology/Neonatologist
