Licensed Practical Nurse (LPN) - Home Health

Location: Chesapeake, Virginia Beach, Portsmouth, Suffolk, and Norfolk, VA
Schedule: Monday-Friday, 8:00 AM-5:00 PM
Mileage Reimbursement: $0.54/mile
Requirements: Active Virginia LPN license

Position Overview

We are seeking an experienced Home Health LPN to provide skilled nursing care to patients in their homes. The ideal candidate has experience with wound care, wound vacs, IV therapy, and chronic disease management, along with strong bedside manner and patient education skills.

Responsibilities

• Provide skilled nursing treatments according to the established plan of care and within LPN scope of practice.
• Perform wound care, wound vac management, IV therapy, lab draws, and other ordered nursing interventions.
• Support chronic disease management and monitor patient conditions.
• Educate patients and caregivers on treatment plans and independent living.
• Collaborate with Registered Nurses and other healthcare professionals to ensure quality patient care.
• Complete accurate clinical documentation, preferably using Homecare Homebase (HCHB).

Qualifications

• Active Virginia LPN license.
• Home health nursing experience preferred.
• Experience with wound care, wound vacs, IV therapy, and lab draws required.
• HCHB experience preferred.
• Strong communication skills and excellent bedside manner.

Additional Information: On-call and overtime may occasionally be required but are unlikely.
