Requirements:

  • Board Certified or TRULY Board Eligible in Surgery-Plastic (must become certified within 5 years)
  • Valid IA license
  • Hold current and unrestricted state and federal authority to prescribe and administer medications as necessary or appropriate to the field of practice
  • DEA
  • BLS Certification
  • Clean malpractice and license history (preferred)
  • Self-Query NPDB pulled within 30 days
