About the Role: Care Plus Hospice Care LLC is looking for a compassionate and dedicated Hospice LPN to join our team in the Phoenix, AZ valley. Responsibilities: - Provide direct patient care and comfort-focused nursing services to hospice patients in home or facility settings
- Administer medications, manage pain, and monitor symptoms under the direction of an RN or physician
- Document patient assessments, care plans, and clinical notes accurately and in a timely manner
- Communicate patient status changes to the supervising RN and interdisciplinary care team
- Educate patients and family members on care procedures, medication management, and what to expect
- Provide emotional support and compassionate presence to patients and their loved ones
- Coordinate with hospice aides, social workers, and chaplains to ensure holistic patient care
Requirements: - Active LPN license in the state of Arizona
- Prior hospice, palliative care, or end-of-life nursing experience required
- Strong knowledge of pain management, symptom control, and comfort care protocols
- Excellent communication and interpersonal skills with patients, families, and care teams
- Reliable transportation and valid driver's license for home visit travel
- Current CPR/BLS certification
- Compassionate, patient, and emotionally resilient demeanor
