Home Health Occupational Therapist Responsibilities: - Evaluate patients' functional abilities and develop individualized treatment plans
- Implement therapeutic activities to improve performance of daily living tasks
- Educate patients and caregivers on adaptive equipment and safety strategies
- Monitor patient progress and adjust care plans as needed
- Document visits and collaborate with the interdisciplinary home health team
Home Health Occupational Therapist Qualifications: - Graduate of an accredited Occupational Therapy program required
- Active California Occupational Therapist license required
- Minimum of one year of Occupational Therapy experience required
- Current BLS/CPR certification required
- Valid driver's license and reliable, insured vehicle required
