Position Overview
In this full-time outpatient role, you'll provide primary care services across the lifespan, working alongside a team of skilled physicians, behavioral health professionals, nurses, and support staff. You will manage a broad range of primary care needs, focusing on continuity of care and positive patient outcomes. The typical workweek involves 35 hours of direct patient care with 5 additional hours for administrative duties. Our clinic's collaborative care model includes integrated behavioral health, enhancing your ability to address complex medical needs.

Compensation & Benefits
- Base salary: $114,716 to $128,481, depending on experience
- $20,000 reward and retention bonus, paid in allotments
- $2,500 annual CME allowance, plus five CME days
- Guaranteed minimum 2.5% annual salary increase

Ideal Candidate
The ideal candidate is a compassionate and professional Family Nurse Practitioner or Physician Assistant with board certification and an active Indiana license. You should have a background in family medicine or primary care, with a passion for serving diverse patient populations. Experience in a Community Health Center or Federally Qualified Health Center is a plus, as is Spanish-speaking ability.
