PATIENT INFORMATION
  • Average Daily Volume: 18-20 patients per day
  • Case Mix: Follow-up visits, new patient evaluations, treatment plans, medication management
FACILITY INFORMATION
  • EMR System: Epic
  • Specialty Backup Available: MD onsite 3 days per week
  • Support Staff Available: Experienced Support Staff
JOB REQUIREMENTS
  • Licenses: Active SC License, DHEC
  • Other Qualifications: EPIC experience required
DUTIES & RESPONSIBILITIES
  • Manage an average daily schedule of 18-20 patients.
  • Conduct follow-up visits and evaluate new patients.
  • Develop and update individualized treatment plans.
  • Provide medication management in accordance with established clinical protocols and applicable regulations.
  • Document patient assessments, treatment decisions, and care plans accurately and promptly.
